Yearly Well Check (Annual Physical)
- These physical exams are done every 365 days for children aged three and older. Until a child reaches three years old, we follow the standard well check recommendations for children, which occur at closer intervals.
- These examinations are performed for preventive care and there is no co-pay for most types of insurances. If there are medical issues that need to be addressed, prescriptions written, referrals made or if there is a new or existing medical condition that requires attention, there may be a co-pay for the visit.
- During these examinations, we assess growth and development, and screen for problems such as anemia, vision and hearing problems, developmental concerns, school and academic issues, and we may address any new or ongoing health problems.
- Any immunizations that your child requires will be administered at these visits.
- This type of physical examination is required for school entrance at all Virginia schools.
School Physical/Child Care Physical
- If your child has had a well child check (see above) within the last 365 days, then that exam will suffice to meet requirements for any school or childcare physical. We will fill out a school or child care physical form based on that last exam. If not requested on the date of examination, there will be a charge for completing any form.
Camp Physical/Scout Physical
- If your child has had a well child check (see above) within the last 365 days, then that visit will suffice to meet requirements for any camp or scout physical. We will fill out a camp or scout physical form based on that last exam. If not requested on the date of examination, there will be a charge for completing any form.
Sports Physical (Pre-Participation Physical Evaluation)
- Pediatric Affiliates of Hampton Roads offers sport physicals for patients. Sport physicals are not the same as a preventive physical/wellness visit as sport physicals focus on clearing a patient to play sports or participate in an activity. Each school, organization, etc. has its own criteria and policies for sport physicals – some facilities require the examination to be performed close to the start date of the sport or activity, others may accept a sport physical as long as it has been performed within 365 days.
- Because of the varying criteria, policies, and forms to be completed, we ask that you refer to the organization’s requirements PRIOR to scheduling a sport physical. We also ask that you provide us with the sport physical form to be completed at the beginning of the appointment. Our facility has a general sport physical form, but more often than not, the organization in which your child is playing a sport for will require their own particular form to be filled out by the physician.
- Since a sport physical is used to clear a patient for an extracurricular activity, it is not viewed as medically necessary. This means that insurance WILL NOT COVER THIS SERVICE. Pediatric Affiliates charges $65.00 for this service and is due at the time of check-in.
- If you need a sport physical and it happens to align with the time your child is due for their wellness visit, Pediatric Affiliates can couple the sport physical with the wellness physical, and as a courtesy, we will not charge you for the sports physical. We ask that you inform us when scheduling the wellness visit AND inform the physician BEFORE the start of their examination that you would also like a sport physical. If the physician is not informed by the parent that they needed a sport physical and the sport examination is not performed, the physician WILL NOT be able to fill out the sports physical and you will have to schedule a separate visit for this service.
Form Completion Requirements
- Forms for all of types of physicals will be completed at the time the physical is done, at no charge, as long as you inform us on the day of the exam.
- If you bring your child for an annual well check and your child is ALSO planning to participate in sports, please let us know PRIOR to the physician’s visit, so we can review the athletic form and then complete the additional examinations needed for a sports physical. (see above)
- If you do not inform us that you need a special physical form when you are here for your child’s well check, and then at a later date request a form, we need to pull the chart and the physician will need to review the history and physical exam. There will be a charge for completing any form after the day of exam.
- If a sports physical was not requested on the date of the annual exam, you will need to make a separate appointment specifically for a sports physical.
